may be it’s the collar you are using, try different types or may be he just hates having a collar. Cheese Burger

There are several reasons why your cat might be scratching. Since you’ve ruled out fleas, you might want to experiment with hypo-allergenic foods. And if he’s wearing a flea collar, remove it. These don’t work well against fleas and you’d be better off with a topical treatment like Frontline.
Itchy cats usually respond well to steroid therapies. Most itching/skin problems can be treated with steroid pills or monthly injections.

Many years ago, my siamese did the exact same thing except the hair pulling was on her back close to the tail. My vet gave her birth control pills for dogs and the itching stopped, her hair grew back, and she became at peace with herself. Maybe that’s an old cure, but it worked. I would get a second opinion with another vet or even a third until you get the help you need for your cat. Good luck. Vicki
Allergies.. possible food allergies, I bet if the VET gave him an allergy shot it would help heal and then you can start to narrow down what it could be, my cat is allergic to cat nip and beef. Even one flea can cause intense reactions in cats who are allergic to them. Use a tightly tined comb to check for fleas or flea dirt. There may be only a few fleas on the cat. Check above the hips, around the neck and shoulders, and on the chest area.
If the cat has a collar, remove it.
I’d change the food to something else, in case this is caused by a food allergy. hudsongray
does your cat wear a collar ? if so sounds like this could be the problem, and you need to remove it. mrs w
