Solving my skin problems?

Now, due to growing up with a host of problems with my skin, most prominent being serious eczma, i have horrible skin.
What i really want to sort are my face and hands. firstly, my hands are horrid. the skin is really dry and flaky, and it doesnt matter what cream i buy and use, nothing really sorts it. what can you suggest?
Then theres my face, first to my lips. my lips have a very strange problem, they seem to have an extra layer. which is dry and sometimes hard. it slowly dries untill it comes up at the edges and then i have to peel it iff (gross i know), this leaves a sore red mark which is why i immediately put lip balm on which leaves my lips nice and soft (and normal looking) for around a day or so and then the process repeats itself. it drives me insane. then to the rest, spots and pores.
i have spots, but they arent bad, they are few and far between but still noticeable. what do you suggest to remove this, (bare in mind i have sensitive skin and even using shampoo makes my head itch)
then, finally, pores. like many people im sure i have ‘blackheads’ or noticeable pores. what can i do to get rid of these?
i just want clear smooth normal skin, sorry for the lecture but i needed to get everything across :).

Having eczema may change some things (like which hand creams you can use - some might really burn so you may have to get cream specifically for this) but at night, I use hand cream and put on an amount so that it’s not all absorbed into my skin and then overtop, I put on a pair of latex/vinyl disposable gloves. I find this really helps my dry skin on my hands.
I’m not really sure about lips. I don’t really have a problem with really dry lips. Possibly because I use a generous amount of Burts Bees Beeswax Lip Balm many times throughout the day. (I find Burts Bees works best, but I’m sure there’s many out there that work just as good.)
For pores, I don’t really have a definite solution. I, too, would like my pores to appear a little smaller. You can try the products out there that claim to reduce pore size but I’ve read that they don’t really work so I have bothered spending my money on them. Though, if it is something you really want, it may be worth it to give it a try. You can get these in the “makeup” sections at most stores. Just ask the lady (usually a lady, anyways) working in the cosmetics department and see what she recommends.
I would suggest washing your face daily with a mild soap. My facialist recommended using Equate Gentle Skin Cleanser or Cetaphil Gentle Skin Cleanser on my face since it is very cheap (Cetaphil being a little more expensive than Equate). I didn’t find using the $30 facial cleanser from the spa worked any better than the $4 bottle of Equate.
There are products meant to getting rid of blackheads as well. You could try those. Again, I haven’t really used those so I can’t recommend how well they work or which ones to try. I do like using those strips that you stick on and then peel off though. Those are cool, lol.
If you can afford it, I would suggest getting a facial every now and then. I try to get one every month but it usually ends up being every 3 months or so. They’re really relaxing and feel great.
