Dog’s cast causing skin problems?

I have a little Pomeranian that broke his leg around 4 1/2 weeks ago. It hasn’t fully healed yet and my vet told me he might take another 4-6 weeks to fully recover. At first I was concern because when I went to the ER the first night the vet there told me to get his cast change every week or two, while my regular vet told me that he will be fine with the same cast on until his leg is healed. Does anyone know who is right? Also, I am worried cause I started to notice him licking and chewing on his cast lately. So, today I took a look at the top part of his leg and his skin seemed to be dry and with red spots. Should he get his cast change to avoid any bacteria or any skin problems? They never prescribe me with any antibiotics to prevent any infections, because they told me he wouldn’t need it. Can someone please help me with my concerns.
