Absolutely, yes, you can have one without any skin signs.
He needs to see a doctor immediately.

Sweetie, I’d have no way of your brother has a Staph infection. He can’t be diagnosed online. They are not usually fatal.
To answer your first two questions, run a search and yes you can have a Staph inefction without skin involvement. It all depends on the site of the infection.
